In the realism department, one must first recognize that professional wrestling tries best as it can to emulate otherwise dangerous maneuvers in a way as to not actually hurt anyone. However, poorly executed moves can lead to rather dangerous situations, a lot of pain and injury, even death. While most are cumbersome for real world usage, I've spent some time thinking about what's actually useful in fighting with regards to ease of use as much as a capacity to injury.
Topping the list is the Spear. A running shoulder thrust to the midsection, a spear can, if hit properly and accurately, deal a great deal of pain on someone, with a capacity for breaking bones and causing internal bleeding. The Spear tops the list due to the fact that virtually anyone can perform the move without much difficulty and it's overall efficiency for taking down an aggressor and incapacitation.
Next on the list is a wrestling classic, the Clothesline. Now, while it is probably inefficient against an opponent who knows basic martial arts, and is in theory a glorified haymaker, a properly delivered Clothesline (i.e. Clothesline from Hell) can, like the Spear, deal a great deal of hurting. A Clothesline can cause anything from a broken jaw to serious concussions if the force behind the blow is hard enough to send the target falling back hard on the back of their heads hitting concrete adding insult to injury.
Running STO, period. Effectively a glorified Clothesline (if that's even possible), the Running STO is a take down maneuver that's both easy and effective. But again, this is assuming you're fighting a complete idiot who knows nothing about martial arts and self-defense disciplines. The damaging aspect of the Running STO is that like the Clothesline, it sends the target backwards onto their backs. Add a little extra oomph to the take down and you you be smashing skull and back into the concrete down below leading to possible concussions and fractures to the skull which will incapacitate anyone. If nothing also, the Running STO allows for easy incapacitation as it makes even an amateur look like a pro to other amateurs in its appearance and flair.
Next on the list, I'll let myself become a little bit more farfetched and add the RKO to the list. Now, how likely are you to actually be able to use this jumping cutter effectively in real combat? Probably not likely if at all most likely resulting in you making yourself look like a tool. Now, if you do manage to pull it off, and it is hardly beyond the realm of possibility, you will, for those three seconds or more, make yourself feel like a wrestling god as JBL would kindly put it. The problem with the RKO is that you are yourself likely to hurt yourself more than you'd hurt the target as you are landing flat on your back, likely smashing the back of your skull while at best breaking your target's jaw. While Orton's Punt is itself a lot more efficient, how many people do you know will assume the position long enough for you to charge and kick their heads off? Unless you friends are willing to kneel down and bow their heads to let you try it out, I say you're better off trying to leap into a semi inverted position, back flat facing the pavement below and hope you can reach around your target's neck or head and have the momentum and strength to drag them down face into the ground or shoulder depending on how you manage to hit it. While cool looking, this is probably very ineffective and cumbersome for actual realistic use. But if you do manage to hit it just right... much pain should ensue.
Last but not least, and probably the most painful move on this list is the classic DDT. In any shape or form, having your head planted straight into the ground must hurt. A lot. While the DDT is not the most execution friendly move on the planet, it seems much easier to pull off than most back breakers, neck breakers, et cetera. All you got to do is clutch your arm around your target's head and do a reversed take down. The least of your target's worries will be a concussion. Add real momentum and a hard surface down below and you can easily kill someone. Not that you'd want to do that, but just throwing it out there.
