No there are plenty of real people who use it. But there are also a FUCK LOAD of people who use it for one reason and one reason only, and that's to troll motherfuckers. I'm ashamed to say even I've done it. You set up a fake webcam with a program, and what you do is either

A) Play a clip/show a picture that is so disgusting that the person you are chatting with will have a cringe-worthy reaction (get that Prntscrn button ready boys!)

B) Play a short "loop" or video clip taken from someone's webcam, preferably a very attractive girl, so it gives the appearance to the person you're talking to that you're a woman. From here lulz can ensue on several different levels.

But yes it's also kind of like a strange Video-*********ion-Chat-Cult...
