From its inception, the Mayhem championship has undergone many plastic surgeries with the rules regarding the title changing multiple times. Whether it was considered an unofficial championship contested under unsanctioned matches, or competing under a 24/7 rule, only one thing has changed - the Mayhem championship has always been defended with No Disqualifications or Count-Outs, eventually creating the signature WZCW match known only as Mayhem Rules and, for some reason, with most of its matches ending in the unlucky loser being put through a table.

Many, many challengers and champions have come and gone throughout its tenure but one things for certain: this belt changes you.
