A Graphics Request

I need a graphic done for me in dedication to Jimmy "The Rev" Sullivan. I have quite specifics to be done, but it shouldn't be too hard.

Each backslash means make it a new line there, so it should be four lines.

I want the size to be somewhere around 400X175 to 450X200
